OFFICE POLICIES

Payment Policy

Co-pays and deductibles are due at the time of service. Insurance billing is provided as a courtesy. If your plan is out of network, full payment is required at the time of your visit.

Missed Appointment Policy

A $50 fee applies to missed appointments or arrivals more than 15 minutes late without 24-hour notice. Three missed appointments within 12 months may result in termination from the practice.

Referrals

If you do not hear from a referred provider within 2–3 days, please contact our office.

After-Hours Care

For medical emergencies, please go directly to the Emergency Department.

Changes of Information

Please notify us promptly of changes to your contact or insurance information.
