Introduction

At Integrative Medical Care (“IMC”), we are committed to treating and using your protected health information responsibly. This Notice of Privacy Practices describes the personal information we collect, how and when we use or disclose that information, and your individual rights regarding your protected health information.

Understanding Your Health Record / Information

Each time you visit IMC, a record of your visit is created. This record may include your symptoms, examination and test results, diagnoses, treatment, and a plan for future care. This medical record serves as:

  • A basis for planning your care and treatment

  • A means of communication among healthcare professionals involved in your care

  • A legal document describing the care you received

  • A means for billing and payment verification

  • A tool for healthcare education, quality improvement, and public health activities

Understanding what is in your medical record helps ensure accuracy and allows you to make informed decisions regarding disclosures.

Your Health Information Rights

Although the medical record is the physical property of IMC, the information belongs to you. You have the right to:

  • Receive a paper copy of this Notice upon request

  • Inspect and obtain copies of your medical information (subject to limited legal exceptions)

  • Request amendments to your medical information if you believe it is incorrect or incomplete

  • Request an accounting of disclosures of your medical information

  • Request confidential communications regarding your health information

  • Request restrictions on how your information is used or disclosed (IMC is not required to agree)

  • Revoke your authorization for disclosures, except where action has already been taken

IMC Responsibilities

IMC will:

  • Maintain the privacy of your health information

  • Provide you with this Notice describing our legal duties and privacy practices

  • Abide by the terms of this Notice

  • Notify you if we are unable to agree to a requested restriction

  • Accommodate reasonable requests for confidential communications

We reserve the right to change this Notice and make the revised version effective for all protected health information we maintain.
